Admissions

It can be stressful to make decisions about the care of a loved one, and even to realize that your loved one has reached a point where more intense care is needed. It may be medical needs that are too great for a family to provide, or personal care needs that are too much to manage.

Our admissions team is here to help.

A skilled nursing facility like Holy Trinity is one option to consider, depending on the level of care needed. Long-term and short-term skilled nursing and advanced rehabilitation care (as after a hospital discharge) are available from Holy Trinity. You can visit any time, but it’s better to call ahead to arrange an appointment. It can be helpful to bring a list of your questions (see the Resources page) to find out whether our facility will be a good match for your loved one. If you are considering Holy Trinity, our admissions director will review all information to see if we are the appropriate place to meet you or your loved one’s needs. The admissions director also finds space on the appropriate unit. If we determine that Holy Trinity can provide the care you need, we will closely coordinate the admission details with you. 

Our team will make the admission process and transition to Holy Trinity as smooth as possible.
